We are very fortunate to have Aaron as one of our Patient Support Workers at Terrey Hills Medical Centre. This is made possible through our partnership with PCCS.

We know that most health outcomes are associated with their social determinants of health and that is what Aaron is here to address. Through our unique partnership with PCCS, Aaron is able to provide valuable support services to our patients to improve their physical, mental and social health and well-being.

More than just a Social Worker, the skills and services that Aaron can provide include:

• Social Prescribing (Social Prescribing enables healthcare professionals to connect patients with social services and groups, addressing root causes of poor health and combating rising loneliness and social isolation.)

• Mental Health Support

• Assistance with Centrelink, NDIS, Aged Care Services, Housing

• Behaviour Change Support

• Carer Support Services
