The care plan is coordinated by your GP/nurse to manage all health care professionals involved in your care. If you have a chronic condition such as diabetes, hypertension, osteoporosis, arthritis, stroke, cancer, chronic pain please enquire with your GP if a care plan may be suitable for you. In addition five Medicare-subsidised allied health (e.g. podiatry, dietitian, physiotherapy, chiropractor, diabetes education) visits a year to support your goal attainment.
