Jess Lee

We are very fortunate to have Jess as one of our Patient Support Workers at Terrey Hills Medical Centre. This is made possible through our partnership with PCCS.

We know that most health outcomes are associated with their social determinants of health and that is what Jess is here to address. Through our unique partnership with PCCS, Jess can provide valuable support services to our patients to improve their physical, mental and social health and well-being.

More than just an experienced Social Worker, the skills and services that Jess can provide include:

• Social Prescribing (Social Prescribing enables healthcare professionals to connect patients with social services and groups, addressing root causes of poor health and combating rising loneliness and social isolation.)

• Mental Health Support

• Assistance with Centrelink, NDIS, Aged Care Services, Housing

• Behaviour Change Support

• Carer Support Services
